I don't know that the cover has ever actually been the deciding factor for me, but I won't lie that the good ones catch my eye and make me curious about what they might be about...which sometimes results in a purchase/read!

Same! I would never buy a book based solely on its cover. If the cover leads me to a compelling plot summary, THEN I'll consider buying it. At the library, I'd probably pick one up based solely on the cover since I have nothing to lose by checking it out.
